Occasionally we take photos of the players solely for the use of the club website and press; you hereby give consent for your child to be photographed solely for such purposes.

In case of emergency, I as the parent/carer of the player being registered hereby authorise medical assistance to be given to my son by a qualified first aider if required whilst I am not available.

In completing this registration form, I have read and agree to abide by the Club's Code of Conduct and the terms of registration given on the website. I understand the all subscription fees and any relevant fines must be paid before my child is eligible to play for the club. I would like my child to be registered with FC United London for the 2024/25 season and understand that all membership fees must be paid and up to date in order for the Player to train and play.
